Telehealth-Enhanced Assessment and Management after Stroke - Blood Pressure
TEAMS-BP is a trial evaluating two comprehensive and evidence-based strategies for managing blood pressure (BP) following stroke. The primary objective of this trial is to compare the effectiveness of Intensive Clinic Management (ICM) versus Intensive Tailored Telehealth Management (ITTM) on achieving systolic blood pressure control (SBP). Eligible and enrolled participants will complete baseline questionnaires and assessments at Study Visit 1 and will be randomly assigned to either ICM or ITTM using a stratified block randomization. Interventions will be delivered over the course of a 6-month treatment period. Medications will be monitored and adjusted as needed to lower SBP to the target using evidence-based strategies as a foundational element of ICM and ITTM.
Blood pressure, patient activation, cognition, disability, and other study outcome measures will be collected at 3 (Study Visit 2), 6 (Study Visit 3), and 12 (Study Visit 4) months.
